Integrating Wellness respects your privacy. This policy explains what personal information we collect, why we collect it, how we use and protect it, and how you can access or correct it.

We handle your information in accordance with the Privacy Act 1988 (Cth), the Australian Privacy Principles, and the Health Records Act 2001 (Vic).

1. Who We Are

Integrating Wellness is a chiropractic clinic at 59 Geelong Rd, Bannockburn VIC 3331.

In this policy, “we”, “us” and “our” refer to Integrating Wellness.

2. What We Collect

Personal information

Your name, date of birth, address, phone number, email address, emergency contact, and, where relevant, your Medicare number, private health fund details, or DVA number.

Health information

Your health history, symptoms, examination and assessment findings, diagnoses, treatment notes, care plans, imaging and test results, correspondence with your GP or other practitioners, and any other clinical information relevant to your care.

Health information is treated as sensitive information under the Privacy Act. It receives a higher level of protection than other personal information, and we only collect it with your consent or where the law allows.

Payment information

Details of payments made to us. Card numbers are processed by our payment provider and are not stored by us.

Website information

When you visit integratingwellness.com we may collect your IP address, browser and device type, pages viewed, time spent on the site, and the site you arrived from. See section 6.

3. How We Collect It

We collect information directly from you wherever possible, including when you:

  • Book an appointment online, by phone, or in person
  • Complete a new patient form or intake questionnaire
  • Attend a consultation
  • Contact us by phone, email, or through the website
  • Interact with our social media pages

Sometimes we collect information about you from someone else, such as a referring GP, another health practitioner, your private health fund, or a parent or guardian where the patient is a child. Where that happens, we will take reasonable steps to make sure you know we have received it.

4. Why We Collect It

We collect and use your information to:

  • Provide chiropractic care and manage your treatment
  • Communicate with you about your appointments, including reminders
  • Bill you and process payments and health fund claims
  • Communicate with other practitioners involved in your care, with your consent
  • Meet our legal, professional, and insurance obligations
  • Manage and improve the clinic and our services
  • Send you information about our services, where you have agreed to receive it

We will not use your health information for anything unrelated to your care without your consent.

5. Who We Share It With

We do not sell your information. We may disclose it to:

  • Other health practitioners involved in your care, such as your GP, a specialist, or an imaging provider, with your consent
  • Our practice management provider, Cliniko, which stores our patient records and booking data
  • Our payment provider for processing payments
  • Health funds, Medicare, DVA, or insurers where you are claiming a rebate or benefit
  • Our professional advisers, such as our accountant, insurer, or lawyer, where necessary
  • Anyone you have authorised in writing to receive your information
  • A person responsible for you, such as a parent, guardian, or carer, where appropriate
  • Others, where required or permitted by law, including to prevent a serious threat to life, health, or safety

6. Our Website, Cookies, and Advertising

Our website uses cookies and similar technologies. Cookies are small files stored on your device that help the site work properly and help us understand how it is used.

We use:

  • Essential cookies, which are needed for the site and the booking widget to function
  • Analytics tools, such as Google Analytics, which tell us how many people visit the site and which pages they read
  • Advertising tools, such as Google Ads and Meta (Facebook and Instagram) tracking, which measure whether our advertising is working and may be used to show our ads to people who have visited the site

These tools collect information about your device and browsing activity. They do not collect your health information, and we do not use any information about a health condition you may have read about on our site to target advertising to you.

You can block or delete cookies through your browser settings. If you do, parts of the site, including online booking, may not work as intended.

Our online booking is provided through Cliniko. Information you enter into the booking form is handled under Cliniko’s privacy policy as well as this one.

7. Appointment Reminders and Marketing

We send appointment reminders and clinical communications by SMS and email as part of providing your care.

If we send you marketing communications, such as a newsletter or an offer, we will only do so where you have agreed to receive them. Every marketing message includes a way to unsubscribe, and we will action it promptly. Opting out of marketing does not stop appointment reminders or messages about your care.

8. How We Protect Your Information

We take reasonable steps to protect your information from misuse, loss, and unauthorised access. These include password protected systems, access limited to staff who need it, secure storage of any paper records, encrypted data transfer, and staff obligations of confidentiality.

If a data breach occurs that is likely to cause you serious harm, we will notify you and the Office of the Australian Information Commissioner as required under the Notifiable Data Breaches scheme.

9. Accessing and Correcting Your Information

You can ask to see the information we hold about you, and you can ask us to correct it if it is wrong, out of date, or incomplete.

To make a request, contact us on 0493 166 999 or at info@integratingwellness.com. We may ask you to verify your identity. We will respond within 30 days.

In most cases we will give you access. There are limited situations where we may not be able to, such as where giving access would pose a serious threat to someone’s life or health, or where the law requires us to refuse. If we refuse, we will tell you why in writing and explain how to seek a review.

A reasonable fee may apply for retrieving and copying records, particularly for large or archived files. We will tell you the cost before we proceed.

10. Complaints

If you are concerned about how we have handled your information, please contact us first on 0493 166 999 or at info@integratingwellness.com. We will acknowledge your complaint and aim to resolve it within 30 days.

11. Changes to This Policy

We may update this policy from time to time. The current version is always published on this page with the date it was last updated.
